Dr. Marie E. Zakrzewski Medical Scholarship

Program Description

The Dr. Marie E. Zakrzewski Medical Scholarship of $3,500 is awarded each year to a young woman of Polish ancestry for first, second, or third year of medical studies at an accredited school of medicine in the United States. Funding is for academic year 2011-2012.

Eligibility

United States citizens of Polish descent and Polish citizens with permanent residency status in the United States who are entering first, second or third year of M.D. studies in academic year 2011-2012 and who have a minimum GPA of 3.0. This scholarship is open to female residents of the state of Massachusetts. The scholarship is non-renewable.

Selection Criteria

Selection is based on academic excellence, the applicant's academic achievements, interest, motivation, interest in Polish subjects and involvement in the Polish American community. Financial need is taken into consideration. First preference is given to residents of the state of Massachusetts. Qualified residents of New England are considered if no first preference candidates apply.

Application Procedure

Application forms for academic year 2011-2012 will be available here from September 2010 through January 4, 2011.

Submit a non-refundable application fee of $35 and the following supporting materials to the Kosciuszko Foundation.  These materials are due at the Kosciuszko Foundation on January 5.

  • Completed Tuition Scholarship application form and Financial Information page.
  • A personal statement. In detail, discuss your academic goals, career goals, and why you wish to pursue a career in medicine. Be sure to discuss your area of specialization.
  • Two passport photos for publication purposes. Print your full name on the reverse side of each. Xeroxed copies are not accepted.
  • Provide proof of Polish ancestry. Use birth certificates, passports, naturalization papers and other documents to show the line of descent from the ancestor who was born in Poland to the applicant. Include a cover page outlining your Polish heritage. If documentation is not available, a letter from a prominent member of the community verifying your Polish ancestry will be accepted. Letters from relatives are not accepted.
  • Official transcripts for the last three full years of schoolwork through Summer 2010. Fall 2010 coursework will not be considered.
  • Two confidential letters of academic reference from professors/teachers submitted on letterhead, enclosed in a sealed envelope and signed across the back flap of the envelope.

All applicants seeking funding towards academic year 2010-2011 via Kosciuszko Foundation's Tuition Scholarship program will be informed of the results of the Scharolship Committee's decision.  Notifications are made in writing on or about May 15, 2010.
